Details

To report a street defect, like a pot hole or a flooded street, use the following form which will be directed to the Street and Alley Superintendent or you may call 217-732-4655.

 

 

1. Enter a Location:

Please enter an address below in the search box. A marker will be placed on the map. You can then drag the Marker to get as close as you can to the location where the defect is.

 

 

2. Enter a Description

Please describe the defect including exact location in the street, width, depth, and your concerns

 

 

3. Your information

Please include your information if you would like to have a response in regards your report. This may include updates on when we plan on addressing the issue or what agency we may forward your request to, as some repairs may be completed by Illinois DOT or a private business. This information is not required.


 

4. Submit your report

If you are complete, check over your information and then submit this report.
